Invitation

The organizer of the tournament – the Foundation of the Czech Society of Hepatology of CMA JEP – beg to invite the registered players – delegates of Prague Hepatology Meeting 2006 to participate in the tournament. It is our greatest aim to prepare an interesting active sports and social programme for the meeting delegates, located in the unique setting of the golf resort of Konopiště chateau; as well as to establish a golf tournament tradition as part of the accompanying programme of future years of Prague Hepatology Meeting.

Rules of participation

• Number of announced registrations: 50.
• Registration is conditioned by the submission of the registration form and registration fee payment. Final deadline is 31st July 2006.
• Individual registrations are to be confirmed accordingly, with regard to the date of delivery of the relevant registration form and the date of receipt of the registration fee by the organizer.
• Deadline for the registration list: 15th August 2006.
• Only one player may register through a single registration form.
• Registration fee: € 25 per one player (including: 1x green fee, refreshments, transfer).

Rules of accompanying persons' programme

• Number of accompanying persons’ registrations: 30.
• Registration is conditioned by the submission of the registration form and registration fee payment. Final deadline is 31st July 2006.
• Individual registrations are to be confirmed accordingly, with regard to the date of delivery of the relevant registration form and the date of receipt of the registration fee by the organizer.
• Deadline for the registration list: 15th August 2006.
• Only one accompanying person may register through a single registration form.
• Registration fee: € 15 per one person (including: 2 hours of golf academy, trainer assistance, golf equipment, space for driving range, training balls, admission to the public course, refreshments, transfer).
