Manual for Registration for the Active Participation and for the Tennis Tournament WMTS 2011
To register for the Active Participation (Presentation Form) or for the Tennis Tournament (Tennis Tournament Aplication Form ) follow these instructions please.

1. Well and completely filled in form ( Presentation Form, Tennis Tournament Application Form) has to be sent by email to the office@congressprague.cz, or by fax to the +420 241 445 806 or by mail to the Congress Prague, Nad Obci I/24, 140 00 Prague 4, Czech Republic.
2. There has to be Your correct email address written in the form. Only with correct email address organizer can confirm acceptance of Your Presentation Form or Tennis Tournament Aplication Form.
3. Each Presentation Form has to be sent together with the Abstract of Your lecture.
4. In case of any questions do not hesitate to contact congress organizer, Congress Prague on the office@congressprague.cz.

WMTS 2011 Event Policy

1. Registration for the congress can only and exclusively be made on-line through the appropriate Registration Form, which is available on the official website of the event
2. Registration fee includes: scientific sessions, tennis tournament, Welcome Reception, National Evening, Farewell Dinner, transportation to the tennis competition and social events from the congress hotels, and the WMTS contribution fee.
3. On-site registration and payment will not be possible.
4. Registration restrictions and rules:
   • Registration for the WMTS 2011 tennis tournament is restricted to: licensed medical doctors in good standing in the Medical Tennis Association of the country in which they reside / practice and their spouses or other significant physicians.
   • Each official WMTS delegate will be responsible for the qualification of registrants from their country.
   • Every WMTS 2011 tennis tournament registrant is responsible for his own health, and he/she participates in the tournament at his own risk.

WMTS 2011 Data Privacy Policy

1. The Event Organizing Committee (EOC) respects the legal rights to privacy of each of the WMTS 2011 participants. The EOC endeavours to comply with valid international legislation on individual data privacy regarding the collection, use, and security of all delivered privately identifiable data (PID), provided for the event.
2. The WMTS 2011 Data Privacy Policy namely consists in the following principles:
   • The EOC collects the PID of the WMTS 2011 participants solely for the organisation of the WMTS 2011 event and for limited publication thereof within the WMTS websites /www.clts.cz/wmts2011, www.wmtstennis.org/
   • The PID consist of: full name, age, specialty, home- and e-mail-address, participants’ photographs, and/or videos.
   • The EOC endeavours to process the PID lawfully, according to the international legislation on data privacy.
   • The EOC will keep these data secure and no longer than necessary.
   • The EOC and subsequently the WMTS will publish photos, videos, and any other PID on the unprotected part of its website or through other publication media only and exclusively on the basis of written consents of the participants concerned.
   • The EOC cannot take over any guarantee on the PID misuse beyond the above described limitations howsoever.
   • The EOC cannot take over any liabilities on content delivered by clicking on hyperlinks within its websites howsoever.

Transfers

Airport Transfers

In case of accommodation in Prague at Diplomtat Hotel Prague, participants can order Airport Transfer from the Prague Ruzyně Airport to the Diplomat Hotel Prague and from the Diplomat Hotel Prague to the Prague Ruzyně Airport.

Price: € 33 per one car (1-3 persons)

Transfer from Prague To Karlovy Vary and back

Prague is 150 km far from Karlovy Vary. Participants can order transfer by car or by microbus from the Prague Ruzyně Airport, Prague Main Train Station – Hlavní Nádraží or from Diplomat Hotel Prague to Karlovy Vary and from Karlovy Vary to Prague (Prague Ruzyně Airport, Prague Main Train Station – Hlavní Nádraží or Diplomat Hotel Prague).

Transfer by Car
Price: € 162 per one car (1-3 persons)

Transfer by Microbus
Price: € 220 per one car (4-8 persons)

Participants can order all transfers through Online Registration and Accommodation Form.
