Prague Hepatology Meeting 2010

Registrační poplatky

In case of on-site registration, there will be an extra fee of € 30,- added to the relevant registration fee.

* Registration forms delivered to the meeting organizer after 15st August 2010 will be regarded as “on-site” registration. In such cases, the relevant registration fee will rise by the extra fee of additional € 30,-.

** To qualify for young doctor / student fee, the applicant’s registration form must be countersigned by the head of the relevant university / hospital department and stamped with the official stamp of the university / hospital. The student / young doctor / trainee must be aged 35 or under. In case these conditions are not met upon submission of the registration form, any such application for young doctor / student fee will be ignored.

Registration fees for regular participants, young doctors, students, and nurses include:

• Participation in the scientific sessions
• Free admission to meeting presentations (Accompanying Exhibition)
• Attending the social Welcome Cocktail on 15th September 2010
• Delegate’s Bag including all printed materials of the meeting
• Book of Abstracts
• Meeting Coffee Breaks
• Meeting Lunches

Registration fees for accompanying persons include:

• Free admission to meeting presentations (Accompanying Exhibition)
• Attending the social Welcome Cocktail on 15th September 2010
• Participation in the excursion “Historical Prague” on 16th September 2010
  (registration sheet available at the registration desk)

Registration fees do not include:

• Gala Dinner „Prague Historical Evening“
  (Thursday, 16th September 2010)

Registration
Registration can be made by one of the following methods:

By Mail/Fax
Kindly complete the Registration Form in the printed version of the Second Announcement in block letters and return it to Congress Prague, Nad Obcí I/24, 140 00 Prague 4, Czech Republic, fax: + 420 241 445 806.

By Internet
Kindly complete the Registration Form, which can be found on these web pages, and submit it by clicking on the „Submit“ button.

Attention:
15th August 2010 is the last possible date for registration and for payment of the registration fee in advance!
After this deadline, only on-site registration will be possible.
15th August 2010 is the final deadline for all payments. Please note that registration and services not reimbursed by this date will not be guaranteed.
In case of on-site registration (registration after 15th August 2010, or registration in person, in the Congress Centre Diplomat Hotel Prague within the running hours of the Registration Centre), there will be an extra fee of € 20,- added to the relevant registration fee.

Young Doctors / Students
Acceptance of a registration in the registration category Young Doctors / Students is conditioned by the participant’s age being 35 years or less (achieved upon the registration date) and their submission of a valid confirmation issued by the school / department / hospital concerned. The confirmation must be submitted along with the registration form for the meeting otherwise the application for the discounted fee will be ignored.
